The Forest County Potawatomi Insurance Department administers benefit plans for all Tribal members and full-time employees of Forest County Potawatomi Community. The Insurance Department began processing Tribal Member health care claims in September 2002 and employee claims in June 2003, this includes medical, dental, vision, and hospital. The FCP Insurance Department also provides insurance coverage for prescriptions and life insurance.

Benefit plans include:

  • FCP Tribal Member Benefit Plan
  • FCP Government Benefit Plan
  • Northern Lights Casino Benefit Plan
  • Potawatomi Bingo Casino Benefit Plan

Administration services include:

  • Customer Service via phone, written and in office walk-ins
  • Processing of all medical, dental, and vision claims for payment.
  • Life Insurance filing and beneficiary determination
  • Eligibility
  • Medical Court Orders
  • Medicare Premium Reimbursement for Tribal Members
  • COBRA and HIPAA
  • Subrogation and Recovery
  • Stop Loss Administration.
  • Short Term Disability for Employees

Business Hours:

Not all departments conduct business during the same hours.

Departments located in the Executive Building, old Executive Building and old Tribal Hall operate 7 a.m. to 5 p.m. Monday through Thursday.
