We are aware thathealth care has become one of the fastest growing industries in the United States.   As a result, we want to assure you that we value your decision to entrust us with your health care dollar.  At Forest County Potwatomi Health and Wellness Center, we are committed to providing you with the best available health care, along with convenient and patient-friendly billing services.  Just as your health care is hoped to be painless, we desire to provide you with payment systems that are painless as well.

We have created this site to help you better understand our patient billing process and to answer some of the most frequently asked questions we receive. If you have specific questions about your statement, please call the Health & Wellness Patient Account/Collections Clerk weekdays between 8:00 a.m. and 4:00 p.m.To better service your needs,  please leave a message with your account number,  your question and your phone number and we will return your call in a timely fashion.

Billing and Insurance can be confusing!

As a courtesy to our Patients, the Forest County Potawatomi Health and Wellness Center Business Office submits your medical claims to your insurance company.

Please have ALL current health plan/supplement identification cards with you when you register for patient services.
