POSITION:               Licensed AODA Counselor – CADC III
LOCATION:            Health & Wellness Center and within the Community
SUPERVISOR:        Behavioral Health Department Director
WAGE:                     Grade 13
HOURS:                    40 hours per week

JOB SUMMARY:            Provide Adolescent Intensive Outpatient AODA and Continuum of Care Services along with assisting Adult AODA Counselor as needed with adult cases.

DUTIES AND RESPONSIBILITIES:

  1. Responsible to provide quality AODA assessments, intensive outpatient treatment to Adolescents using the Matrix Model and will work with affected family members.
  2. Responsible to ensure that services are provided in a timely manner that meets all applicable regulatory licensure and accrediting regulations.
  3. Responsible for client intakes, assessments, treatment and discharge planning, including a continuum of care or after-care group for discharged clients.
  4. Participate in multi-disciplinary staffing.  Work in cooperation with Behavioral Health Department regarding planning community in-services, workshops, and programs.  Familiar with all HSF75 Statutes.
  5. Responsible for timely and accurate documentation which meets applicable regulations.
  6. Responsible to ensure outreach, education and prevention for the Forest County Potawatomi Community.
  7. Responsible for crisis intervention, and to coordinate inpatient treatment care when needed.
  8. Must make referrals when appropriate.
  9. Maintains strict confidentiality according to applicable laws and regulations.
  10. Exemplifies personal conduct style, which is reflected in an appropriate role modeling.
  11. Upholds the Professional Code of Ethics established for AODA counselors.
  12. Maintains a cooperative relationship with employees, medical staff and others.
  13. Adheres to and supports facility policies, programs and activities.
  14. Demonstrates tact, courtesy, and a positive approach to communication and interaction with other employees, visitors, physicians, etc.
  15. Responsible for personal development to ensure current knowledge in the profession.
  16. Perform any other job-related duties as specified by supervisor.
  17. Responsible for maintaining a safe, clean and drug free environment.
  18. Implements. Performance improvement measures to continuously look and measure progress and ways to improve quality of care.
  19. Willingness to work beyond what is required periodically and travel for continuing education hours.

QUALIFICATIONS:

  1. Maintains proper certification and licensure that is up to date with accrediting standards for State Certified Mental Health Programs and or State Certified AODA Programs.
  2. Experience with a tribal community is preferred but not required.
  3. Experience in providing AODA services with adolescents is preferred.
