The Forest County Potawatomi Health & Wellness Center offers a variety of Health Funding programs. All of the programs require an application process to be completed with the Health Funding Specialist, located within our facility. We care about our clients being able to access and receive quality health care. The Potawatomi Health & Wellness Center is here to meet the needs of your family.

The Health Funding Office is located on the lower level of the FCPHWC and is open Monday - Friday, 8 a.m. to 12 p.m. and 1 to 4:30 p.m.
