Please note:
Effective January 1, 2009, individuals enrolled in county health care plans between December 1, 2008 and December 31, 2008 were transitioned into the BadgerCare Plus Core Plan.
Applications to the Core Plan will be accepted starting June 1, 2009. The earliest enrollment date will be July 1, 2009.
The BadgerPlus Core Plan will provide access to basic health care services, including primary and preventive care, and some generic drugs to low-income adults. Income-eligible adults from age 19 - 64 who do not have children or do not have dependent children under age 19 living with them, may be able to enroll in the BadgerCare Plus Corp Plan.

Eligibility differs for each of the programs listed above. We hope to have a program available for anyone requiring assistance funding their health care needs.All programs require an application and approval process prior to the date of service, therefore we encourage anyone to review listed information or call our Health Funding Office to have your situation reviewed for assistance.
The following are some basic eligibility program guidelines:
Contract Health- Is open to Enrolled Forest County Potawatomi members and their minor (dependent) children who are under the age of 19, living in either Forest, Marinette or Oconto counties. Services under this program must be referred from the FCPHWC for payment consideration. Clients may have other insurance coverage.
Medical Relief Block Grant- Is open to Enrolled Forest County Potawatomi members and their minor (dependent) children who are under the age of 19, living in Wisconsin and receiving services at the FCPHWC. Recipients of Wisconsin Medical Assistance are not eligible for this program.
Family Assistance Plan- This program is designed for low income uninsured clients receiving services at FCPHWC. The program is based on up to 200% of the Federal Poverty Guidelines set each year by the federal government. Clients can not be eligible for any state or federal program assistance to qualify.
Drug Assistance Program- Is designed to ensure that low income, uninsured clients receiving care at the FCPHWC obtain routine medications at little or no cost. The Health Funding Office will work with you and your provider to identify your specific needs.
Alternative Care Pilot Project- A Managed Care Plan, designed to assist Native Americans in obtaining preventive and chronic medical health care services at the FCPHWC. Each plan has designated services that are covered under each plan, therefore not all services may be covered (depending on your needs).
Direct Service - Is open to Enrolled Forest County Potawatomi members and their minor (dependent) children who are under the age of 19, living in Wisconsin and receiving services at the FCPHWC. Program does require a one time signature form (unless additions or deletions) which require the Forest County Potawatomi members signature to up-date.
