These services are provided to Potawatomi Tribal members and other Native American individuals (depending on the service). These include inhome nurse and community health representative (CHR) visits, nutritional services, domestic violence support and counseling, infant services, immunizations, formula program, elder services, WIC program.

Community Health is a department dedicated mainly to the members of the Forest County Potawatomi Community.

Some of the providers and some programs are available to the general public, for example the Diabetes Program, and the Registered Dietitian may be seen.

For the tribal community, the Community Health program offers services for Domestic Violence, Maternal Health, Stop Smoking programs, the Honoring Our Children Program.

The present Health & Wellness Center grew out the the efforts of Community Health to improve the health of the Potawatomi people.
